CMFA COMPLIMENT/COMPLAINT FORM

COMPLAINTS AND COMPLIMENTS POLICY

The Clarington Minor Football Association strives to ensure a positive experience for all members where open communication is encouragedEach team will have a Team Manager, whose role includes being a “go-between” for the coaches and the parents.  When a coach identifies a player who, for whatever reason, is experiencing problems, the coach will address his/her concerns (and what action should be taken) with the Team Manager, who will communicate with the parents.  Similarly, should a parent have any concerns about their child’s football experience, they should express those concerns to the Team Manager (not directly to the coach), who will then liaise with the coach for resolution.

If the Team Manager thinks that the coach should meet directly with the parent, at a time agreeable to all parties (at least a one day cooling off period where deemed necessary).  The CMFA would request that all complaints be submitted in writing via the attached “Compliments and Complaints” form. If it is deemed at this time that a satisfactory solution cannot be reached by the parents and coach, then it will be directed to the Executive Board of Directors for review and a final ruling.

Please note the following:

• The Clarington Minor Football Association cannot guarantee complete confidentiality. The contents of this document may be shared in an effort to resolve the complaint here within. By completing the form, you agree that The Clarington Minor Football Association may share some or all of this information in the process of resolving the complaint.
• Complaints will be addressed according to severity, resources and safety for participants.
• All complaints must accompany the Clarington Minor Football Association  “Compliment and Complaint Form” prior to review.
